The Person Escort Record (PER) is a set of questions that aims to capture detailed information about a person that is being moved. This includes personal details, risk and health information as well as property details. This record also includes any events that are captured during the person’s move as well as handover. The purpose of the PER is to share critical information with the parties involved in moving and receiving the person to ensure the safety of the person, other prisoners and everyone else involved in moving and caring for them.
