Liquidity Removal dead UI

Note: I am creating this issue as the main entrypoint for the problem to track its progress and link related issues.

Description

In some circumstances, on the liquidity removal section (/liquidity/remove), the form is not responsing, balances aren't shown, nothing is possible to do with it. Only page refresh fixes the situation.

Steps to reproduce

After some research, these steps are found to produce the bug stably:

• Load the app at the Trade section, but not on the Liquidity Removal section (i.e. at /swap, /liquidity or /liquidity/add routes)
• Navigate to any other section (e.g. "Assets", "Earn" etc)
• Navigate back (e.g. by clicking to the "Trade" link in the nav bar, or by "go back" browser feature)
• Go to Liquidity Removal, for any asset
• Enjoy the bug
