docs: link CHANGELOG to relevant issues

- `CHANGELOG.md` now has working links to relevant issues.

  I replaced the issue numbers in `CHANGELOG.md` with the Python script
  available from the following repo:
  https://github.com/kikuomax/link-commit-hash
