  • General: the tables controlling when Derived EOD 2018 Stage Group [818] utilizes the AJCC ID value. These tables had not been updated to reference the new AJCC IDs for the V9 schemas. This has been corrected. If you calculate the Derived EOD 2018 fields, It is recommended to run the derivations across all January 1, 2018 and later cases to ensure the correct Derived EOD 2018 Stage Group is captured. #108

Pediatric conversion tables

In the prior release of the Pediatric 1.1 algorithm there were conversion tables that should have been included. By default the library strips out tables that are not used in any schema. We now also include and table with an identifier that starts with conversion_. See #104 for details.

The field testing version of this API was identified as Toronto Stage and the final version field testing version was Toronto v0.5.
Thanks to all the hard work and thoughtful responses by the Kentucky Cancer Registry, who graciously volunteered to perform the field test for us, and by the countless others who reviewed and contributed to the API, significant changes were made to the system.

The extensive scope of these changes has resulted in a greatly improved Pediatric Data Collection System.

However, since these changes included some paradigm shifts, an audit log of changes between Toronto v0.5 field testing and Pediatric v1.0 Pediatric Data Collection System is not practical.

Some high level changes are presented here.

  • System name changed from Toronto to Pediatric. Field names which included the word Toronto were changed to be Pediatric
    • Toronto Version Number [9620] is the sole exception. This field captures the version of the Toronto staging upon which the Pediatric Data Collection System is currently based.
  • The age range included in those schemas limited by age changed from 0-19 to the broader 0-39 to be consistent with the NCCR age definitions
  • Where appropriate, the capture of Pediatric staging system components moved from distinct SSDIs into the Pediatric Primary Tumor, Pediatric Regional Nodes and Pediatric Mets fields
  • Calculations based on EOD 2018 fields were removed from the Pediatric Schema stage group algorithms
    • As an aid to those registries where EOD 2018 is collected, conversion tables for Pediatric Primary Tumor, Pediatric Regional Nodes and Pediatric Mets have been added. These tables, which are outside the Pediatric schemas, ingest the Pediatric Schema ID, EOD 2018 Schema ID, and EOD 2018 fields and, where possible, provide the corresponding value for the Pediatric field. Not all combinations have conversions. These tables are provided to reduce the need for double coding when the value of the EOD 2018 fields captures the same meaning as the Pediatric field.
  • Those SSDIs included in the system were reviewed and SSDIs now captured in Pediatric Primary Tumor, Pediatric Regional Nodes and Pediatric Mets were removed, while some additional SSDIs of interest were added

EOD v3.1 Review and Conversion:

After EOD v3.1 has been deployed

  1. AJCC ID [995]: there were changes to the V9 AJCC ID values to match the AJCC's new numbering scheme. Logic provided for those who CANNOT recalculate across their database.

    • If Date of Diagnosis Year is 2018 or later, for the AJCC ID [995] within each specified Schema ID [3800], the new value is provided:
      • If AJCC ID = 52 and Schema ID = 09520 (Cervix Uteri) and TNM Edition Number = 9 or blank, then set AJCC ID = 9001
      • If AJCC ID = 19 and Schema ID = 09190 (Appendix) and TNM Edition Number = 9 or blank, then set AJCC ID = 9002
      • If AJCC ID = 21 and Schema ID = 09210 (Anus) and TNM Edition Number = 9 or blank, then set AJCC ID = 9003
      • If AJCC ID = 72.1 and Schema ID = 09721 (Brain) and TNM Edition Number = 9 or blank, then set AJCC ID = 9004
      • If AJCC ID = 72.1 and Schema ID = 09722 (CNS Other) and TNM Edition Number = 9 or blank, then set AJCC ID = 9004
      • If AJCC ID = 72.1 and Schema ID = 09723 (Intracranial) and TNM Edition Number = 9 or blank, then set AJCC ID = 9004
      • If AJCC ID = 72.1 and Schema ID = 09724 (Medulloblastoma) and TNM Edition Number = 9 or blank, then set AJCC ID = 9004
      • If AJCC ID = 72.2 and Schema ID = 09724 (Medulloblastoma) and TNM Edition Number = 9 or blank, then set AJCC ID = 9005
    • OR Recalculate to derive the AJCC ID across the database 2018 and later
    • No other changes are necessary

  2. In situ: there were changes to the calculation tables for the Derived EOD 2018 fields in approximately 41 schemas where AJCC does not define Tis. The Derived EOD 2018 N and Derived EOD 2018 M will both be set to 88 for such cases. Logic provided for those who CANNOT recalculate across their database.

    • If Date of Diagnosis Year is 2018 or later, if EOD Primary Tumor = 000 AND Derived EOD 2018 T = 88
      • If Derived EOD 2018 N is not 88, set Derived EOD 2018 N = 88
      • If Derived EOD 2018 M is not 88, set Derived EOD 2018 M = 88
    • OR Recalculate to the Derived EOD 2018 fields across the database for 2018 and later
    • No other changes are necessary
    • (This can always be applied. Calculation tables were updated. No review necessary.)
  3. Grade: there were changes to the calculation tables for the Derived EOD 2018 Stage Group in 8 schemas where Grade is considered for stage group determination. The calculation no longer prioritizes by timing, but now uses the highest grade captured between Grade Clinical [3843] and Grade Pathological [3844]. Also, for Retroperitoneum and Soft Tissue Trunk and Extremities, the Grade value of ‘H’ was added to the Grade tables in v3.0 but had not been added to the Stage Group calculation tables – this has been corrected. If Derived EOD 2018 Stage Group is collected, it will need to be recalculated.

    • If Date of Diagnosis Year is 2018 or later, if Schema ID [3800] in 00190 (Appendix 8th), 09190 (Appendix V9), 00381 (Bone Appendicular Skeleton), 00480 (Breast), 00430 (GIST), 00580 (Prostate), 00440 (Retroperitoneum), 00410 (Soft Tissue Trunk and Extremities)
      • Recalculate to the Derived EOD 2018 fields across the database for 2018 and later
    • No other changes are necessary
    • (This can always be applied. Calculation tables were updated. No review necessary.)
  4. Grade: Derived Summary Grade [1975] has been defined. This field will be calculated at the central registries for all cases diagnosed 2018 and later. The more severe value from Grade Clinical [3843] and Grade Pathological [3844] will be used. Breast is a special case because behavior affects the priority. If this field is required for your registry:

    • If Date of Diagnosis Year is 2018 or later
    • No other changes are necessary
    • (This can always be applied. No review necessary.)
  5. SEER Site Specific Factor 1 [3700]: the field is changing from a 1 digit fields to a 2 digit field. There is a conversion between the old values and the new.

    • For the Schema ID [3800], the conversion between the old and new values of SEER Site Specific Factor 1 is provided:
    • If Date of Diagnosis Year is prior to 2018, set SEER Site Specific Factor 1 to blank.
    • Else If Date of Diagnosis Year is 2018 or later

    • LVI: LVI was removed from all schemas. The values shown had been limited to those applicable to the schema but a single lookup (available in NAACCR) can be used.

    • Version 9 Updated Diseases: AJCC released the 9th revision of the NET (excluding Adrenal Gland) and Vulva diseases which will be effective for cases diagnosed 1/1/2024 or later. There are now 2 schemas for each.

  • CNS Involvement [9602]: Note 2 and all codes changed so that a physician's documentation of CNS Involvement should be captured in text, but not used to code the data item; Codes 2 and 3 adjusted for bullet spacing and to include equal (<= or >=) in the breakpoint descriptions
