  • updated dependencies (resolves most audit warnings - see below)
  • eliminated the last of the mutation test issues
  • improved code coverage
  • no longer process.exit(1) if the server fails to start.

Note There is still one outstanding audit warning due to an issue in Mocha. Ref mochajs/mocha#3877

I won't merge into master until the mocha issue is resolved.
