Refactor hooking ActiveRecord migration tasks

- Use Rake::Task#enhance insteaad of defining same tasks again

- Remove hooking db:migrate:change task which doesn't exist

- Fix hooking db:migrate:reset task so that the annotation runs after
  all migration tasks (ctran#548)
