Description

A simple text field specifically for collecting the last four numbers of an American social security number with Gravity Forms. The text field includes HTML5 pattern attribute and basic validation to ensure 4 numbers are entered.

On the form, the field will display as: XXX-XX-[input]

Important: This data is not encrypted in any way. So handle with care.

Installation

  1. Upload the gf-field-ssn folder to the /wp-content/plugins/ directory
  2. Activate the plugin through the 'Plugins' menu in WordPress
  3. Add the field to a Gravity Forms form!
