[AF3]/[AF4] UX Improvements

Improvements

1. Hide or Remove the "Delete Title" checkbox from the form.
   
   This setting is a default Drupal setting, and it is not being used on Activity Finder 3 pages (it is used in AF4). It is not obvious to the end-user that this checkbox can be ignored.

2. Hide or disable field for selecting block.
   
   This field always contains a default value, and the paragraph won't work if you select another value from the list. For the end-user, this field is completely useless, and it makes sense to hide or at least disables it.

This point 1 is actually implemented in AF4 - it does affect the starting page of AF4 - you can edit this title and hide it if needed.
The 2nd point is fine, there is this linking to block in AF4 too - so can be improved in AF4 too.
