After a quick check, it seems that the issue comes from the new check-labels workflow. There are multiple instances of it shows up on the example PR https://github.com/pytorch/pytorch/pull/88504/checks and one of them requires approval from maintainer https://github.com/pytorch/pytorch/actions/runs/3395637495
