Add expand all button to tree view

When building or viewing a codelist via the tree view, I often want to expand all of the codes in the tree. This currently requires clicking through all of the expand buttons. It would be nice to have an "Expand all" button that expands all codes in the tree. It may also be useful to allow expansion of all codes within each of the top level concepts.
