Currently --no-reload is the default Flask debug behavior. I can manually remove the argument and hot reload works again, so far I did not observe any issue. What is the rationale of setting --no-reload by default?
Launch the Flask application with VSCode default debug config:

Modify app.py and observe that there is no hot reload
Delete "--no-reload" from the debug config file and the debugger starts hot reloading without issue
