Add entry revisions

Entries should support having multiple revisions. Revisions are essentially drafts, with only one revision at a time being able to be designated "published". You should be able to branch a new revision from a different one -- this will be useful if it is necessary to work on an edit to an already published entry without publishing an unfinished edit.
