With our implementation of this, it does not ask the user to provide their OTP every time they log in. In testing we are logging in with the password and OTP and then logging out again. Upon the second attempt to log in there is no request for OTP

What version are you using? I'm on v4.x with Rails 6.1 and it's working fine, but haven't tried v5 yet. Do you have any custom logic not related to the 2FA implementation that might be conflicting with it?
