Add ability to add context to any question

Users in departments would like the ability to add a comment or note to any question in the AIA. Sometimes, the "yes/no" nature of the question does not let them express the full nuance of the situation, and they would like an opportunity to share additional detail with the reader.

As a potential design, there could be a small button/icon next to each question to add a note, which would open a text box to enter the information. The information entered would be displayed in the results page, including the PDF and the saved JSON. The translation for this text would be entered on the results page.
