ツール選定（Google Formが良さそうだけど）
フォーム作成
フォーム→GitHub Issueへ自動登録するワークフローの登録
エラー画面の連絡先を切り替え
フッターにもお問合せ欄を追加する

メモ

Twitterでの問題は以下の点で問題があると感じたので、別の媒体に切り替えたい

メッセージが個人のアカウントに紐づくので、報告が躊躇される場合がある
そもそもTwitterをやっていないと連絡ができない
文字数が制限されるのでエラーの詳細がわかりづらい
